דף שער לפנקס חשבונות של בית הכנסת העיראקי לשנת 501 להג'רה (1107/08 לספירה), שהוגש למבורך בן סעדיה אלאסראאילי אלמתטבב ראייס אליהוד (=מבורך בן סעדיה), נגיד היהודים והרופא. בצדו האחר של הדף מופיע טקסט ספרותי שבין שורותיו רווחים רחבים. המידע מבוסס על כרטיסיית גויטיין.
