רישום מבית הדין, בעברית, שנכתב ונחתם על ידי יחיאל בן אליקים (פעיל בשנים 1213–1233). מתוך הטקסט: "כשיצא היתום... יבחנוהו האומנים... מה שכתוב בשטר ש... המלמד הזה ובית הדין... ורשמנו את אשר נעשה... כמבואר לעיל". המסמך עוסק ככל הנראה בענייני יתום שנמסר ללימוד מלאכה אצל אומנים, כשבית הדין מפקח על ההסדר ומתעד את שנעשה בכתב. המידע מבוסס בחלקו על כרטיסייתו של גויטיין.

"When the orphan went out... the craftsmen will examine him... what is written in the document that... this teacher and the court... and we have recorded what was done... as explained above."
