קטע המכיל מרשמים רפואיים. אחד מהם מיועד ככל הנראה למאבק בנדודי שינה או בחרדה (קלק). האדם או האנשים שרשמו את המרשמים לא זוהו, וגם לא המקור שממנו נלקחו המרשמים. ניכרים לפחות שלושה כתבי יד שונים בקטע. בצד הוורסו מופיעות מספר שורות של ניסויי קולמוס. (המידע על פי סוואג'-סמית')
Princeton Geniza Project description: Fragment containing prescriptions (recipes). One of them is apparently for combating sleeplessness or anxiety (qalaq). The person or persons jotting down the recipes have not been identified, nor has the source from which the recipes were drawn. At least three hands are evident.
