מסמך בכתב קנצלריה, מרווח שורות רחב, שמורים חלקים מארבע שורות מאי שם לקראת אמצע העמוד מלמעלה למטה. שוליים ימניים רחבים ושוליים שמאליים שמורים שלמים. הטקסט חצוי באלכסון, כנראה כשהושלך (ואחר כך נעשה בו שימוש חוזר לטקסט בורסו; ראה רשומה נפרדת).
Princeton Geniza Project description: Document in a chancery hand, wide line spacing, parts of four lines preserved from somewhere toward the middle of the page top to bottom. Generous right margin and left margin preserved intact to judge by the word-stacking. Text has a slash through it, presumably when it was discarded (and then reused for the text on verso; see separate entry).
