נוסח של שבועה שעל אמו של אבו אלחסן להישבע לו, בעניין שפחה ורכוש נוסף, ובמיוחד בשמים ותבלינים. השבועה נדרשה לאחר מותו של אחיה. בצדו השני של הדף מופיע שיר על שרה אשת אברהם. כל הטקסט נכתב בכתב ידו של נתן בן שלמה הכהן. (המידע מבוסס על כרטיסיית גויטיין.)
Princeton Geniza Project description: Formulary of an oath to be given by the mother of Abū l-Ḥasan to him, concerning a female slave and other possessions, especially spices; oath was necessary after the death of her brother.
